Microsoft Office 2010 has MS Word templates. Open MS Word, create a "New" Document and select from the templates - Book/Other Books/Cookbook.

At work, we've used Morris press and for a private fundraiser we've used cookbook publishers.com.
Like Amom2 said below, that is what my friend did. She made a cookbook, and had it spiral bound it too.
Laminated cover etc.
It was cute.
We started to use 'createmycookbook.com' for a family cookbook. I liked it because there was no big minimum number of books you have to order. I like spiral bound ones that we could add to later. But we ended up going the shutterfly route, it looked way nicer. Not free though!
My hairdresser just put together a cookbook, and I helped her with it. It was very easy to input the recipes and correct them if there was an error, and it was just easy to navigate around while doing it all. She used Morris Cookbooks www.morriscookbooks.com. I think she had to order either 200 or 300 per order, and she was able to sell them for $10 each, and there is 152 pages in the cookbook, just in case you'd like that info too!! Have fun!
