Doodle Doos on Hudson in W. Village. Kids love sitting in special kiddie seats with cool stuff at each station. Cute shop, too.

I don't know where you are located, but I use David Alan cuts for Kids in Paramus (Rt 4, next to Kohls). They have video games or videos for the kids to watch during haircuts and a train table for before and after.

In what city? In my town of ridgewood there's sharkey's cuts for kids which is awesome!

If you are from NJ maybe you can try the place I took my daughter for her first haircut. It's in Fair Lawn on Route 4 East a place called Charlies Kids Salon. If you interested ###-###-####.

I've taken my son to Snip-Its in Rockaway Mall since he's been 5 months. He likes going there and the staff is very friendly and fast. Great for young children. No appointment necessary. It's not inside the Rockaway Mall. It's actually in the Target/Pier One/Dick's outside shopping area.
