If barrier creams like desiring are doing nothing for him... I would suspect a yeast infection.

My son was prone to yeast infections... Especially when his skin was broken (from diarrhea, or his allergy to wipes, etc.).

Diaper rash severe enough to cry over (come to find) is nearly alway a yeast infection. (Not just my kid).

You can use OTC yeast infection cream (also works as barrier cream), athletes foot spray (also works as baby powder)... But I preferred Rx'd cream when it got really bad. .

Destin and Triple Paste have many of the same ingrediants, so if Destin isn't working Triple Paste may not work either.

When my little guy had sever diaper rash, our ped recommended Paladin. Worked like a miracle!

Triple Paste has worked the best when my little ones got really bad diaper rashes. I have 2 with sensitive skin so we had this a lot.
Instead of wipes that sting like crazy use a wash cloth and warm water. When it got really bad I would actually stand them in the tub and use the removable shower head arm to rinse them off then pat them dry before putting a new diaper on.
Desitin and A&D never helped and actually stung worse. Butt Paste didn't always help when it got that bad either. You really have to put the Triple Paste on thick and everywhere and put it on with every single diaper.

Let him soak in a warm (not hot) bath.
Then, use a cotton ball to apply maalox to his bottom. Yes, that Maalox. It is like water, so you will need to put a towel or something under him when you apply it. After application with the cotton ball(s), let him air dry the best you can.
Use soft washclothes and warm water for diaper changes, not wipes, then apply the maalox.
I learned this from my nurse SIL when my little guy had that around that same age. The diarrhea is dumping acidic juices on his bottom every time he "goes". The maalox is an antacid that counteracts it before it can begin to burn his bottom.

ETA: I would add that I would guess yeast infection EXCEPT that he has had diarrhea. With diarrhea, I would assume it is the diarrhea that is causing it, not yeast. You don't want to just coat his bottom with something to keep the acids from the diarrhea to getting on him, you want something that will change it so that it doesn't do more damage. Maalox. Trust me on this.

We just dealt with this also. Try a bath with baking soda and put coconut oi on their bottom. Works wonders.

Happened to my son, put diaper cream on first then get an athlete foot fungus cream and put over the other cream. This is what dr told me and it cleared up fast, like a day or so.

My girls were allergic to Desitin...try A&D or vaseline with cornstarch. Also use a washcloth to clean him; the wipes could be further irritating his skin.

I suggest that a paste or ointment keeps air away from the lesions and what they need to heal is air. If it's a yeast infection, and the ER would've suggested that if it is, then he needs an anti-fungal cream or spray. You'll find it in the foot care section of the store.

I would try leaving his diaper off and not putting a cream or especially an ointment on the skin. Wash with a warm wet soft cloth. Wipes will sting.

My daughter puts olive oil on her baby's bottom at the first sign of redness.

If you're using cloth diapers, try using disposable ones until the rash heals and change them at the first sign of wetness.

I agree. It's yeast. Use no chemicals ie wipes etc and ask a pharmacist for any OTC yeast med. also use the highest zinc content cream u can find.

Desitin works wonders for diaper rash but since it's not working it sounds like either a yeast or fungus infection try a OTC med. This should work. I used the OTC 7 day treatment med for my dtr (she might of been the same age too) & only needed it for 3days but used it for 7days just in case it came back.

Yeast infection is my guess.
If this is the case, then a prescription cream has to be used. AND if the Mom is breastfeeding, she also has to get treated. Otherwise, the yeast infection will just go back and forth between them and it won't go away.

I agree with victoria. We make our own diaper cream and it has been a god send. We combine desitin (or any diaper cream), liquid antacid, and corn starch. It makes a white paste and when applied to the rash, it seriously clears up in a few hours. The antacid helps with the acidity of the urine and feces that burn the area, and the corn starch helps to keep the area dry. Its a wonderful mixture; I recommend it !

Don't worry Grandma, there is a wonderful product sold over the counter called Triple Paste. (Walgreens or CVS) My son was on antibiotics for a while when he was a baby which caused diahhrea often, and the rashes never went away with Desitin. Triple Paste was the only thing that worked and quickly. It was recommended by my pediatric advise hotline when I knew nothing else to do for his poor little butt. I hope it works wonders for your Grandbaby as it did for my son.

Try not to put anything on it with fragrance in. Most diaper rash creams (Desitin, Boudreaux' Butt Paste, etc.) have it.
Triple paste is the best thing I have found for it. Quite expensive but really worth it. I have found significant improvement after one use. Also air dry as much as possible. Poor baby, good luck and hope it heals soon. If it doesn't see the doctor.

Lotrimin
The rash is caused by a fungus and this stuff helps.
After BM, clean baby up really well. Apply thin layer of Lotrimin all over then a layer of Desitin. Do this every time and it'll go away pretty quick.
Good luck!

try Bag Balm, comes in a green, square tin; I know Walgreens carries it, probably can be found at other stores also.

With bad diaper rash, I alternate between Desitin (or the like), hydrocortisone cream, and clotrimazole cream (yes, the stuff for fungus on your feet). Use a different one at each diaper change. A friend learned that trick from a nurse and passed it on, it's worked great for my babies.
