Your brother needs to go to a doctor. Blood can occur in the urine after a marathon, but the blood at night and other times are a concern. He should get this checked out, as the causes can be serious.
Recently I had a conversation with my brother who is training for a marathon and he told me he is experiencing a small amount of blood in his urine. He has also found small spots on his sheets and boxer shorts. Of course since he is my dear family I am a bit concerned...have any of you mommy's who maybe runners or know of any runners who have experienced this? I have been online since he and I talked and found that strenous exercise especially running long distances can cause some blood in the urine as can supplements. I still worry and have advised him to see his dr but you know guys... thanks for any insight or suggestions!

ANY time there is blood in the urine, it's time to call the physician. It doesn't matter if it's from rigorous training, it's impossible to tell the source without going to a doctor, and it can be the sign of serious issues such as kidney cancer.
It can also be a sign of a kidney infection, UTI, kidney stones, etc.
As much as I'd like to answer from a running point of view, I think it's best to advise a call to the doctor and let them determine if he needs to be seen and treated for something more serious.

Strenuous exercising can cause lots of build up, which can back up in the kidneys if a person isn't drinking enough pure water to flush their system out. Does he/has he complained of any pain in his side, or pain when urinating (would lead to suspicion of stones)?
Also, if he gets diagnosed with a kidney problem (stones, uric acid, whatever), a good way to ensure that he never has another problem or attack would be to drink a bit of pure lemon every day, whether he can suck down half a lemon wedge (I know, makes me pucker just thinking about it) or mixing some pure organic lemon juice in water with a little sugar. But straight lemon is the best.
I try to have a little lemon every day. It's very good for the body. Cleans everything out.
He may have kidney stones. The same thing happened to my boyfriend. When he would exercise vigorously (also training for a marathon) he would have blood in his urine. Turned out he had a huge kidney stone.
He needs to call a doctor. Blood in the urine is not a normal occurrence. All they will have him do is pee in a cup. There's nothing invasive about it.
