You should be able to contact your son's coach and ask him for one or where to get one. Also, here is a website that might be helpful, but I'd call your son's coach first.

Ahhhh... the standard "Injury, maiming, & death" form.

They vary city by city, county by county, & state by state. They're usually worked out with a lawyer to specifically meet that local's reqs... and are part of a business expense,

I, too, am confused. Do you mean a form that allows the coach to get medical treatment for your son if you're not available? Or do you mean a form that allows the coach to get information from the doctor that states he's medically fit enough to play the sport? I think both of those are provided to you by the team. Part of my confusion is that the forms should have been included in the printed information given to you when you asked to sign up your son for the team.

The doctor may have his own form that will state the he's fit to play.

You can write your own note that gives the coach the right to obtain medical treatment for your son in the event that you're not available. That is all that it has to say. Or you can narrow the options to for example only when the injury is related to the times that your son is under the supervision of the coach.

I don't understand the question, sorry, it's been a really long day. Can't you just go to their web site and download one? Or get the commissioners phone number? Or call the coach? What is a medical release form? I guess that's the part I don't understand, I know we sign stuff at the beginning when we sign them up for sports but I don't remember what all there is.

My daughter is in cheer and orchestra. The cheer coach gave us a form to fill out and be notarized and the orchestre director gave us a form to have filled and notarized for the upcoming spring trip this weekend.

A generic form could probably be located at your school.

Ask you pediatrician. If you don't have a regular doctor, then a sports physical might be necessary. Those are available in many places.
