Authorization to Consent to Medical Care

Just wondering if anyone knows where I can get an "Authorization to Consent to Medical Care" form for my children. I would prefer to print it out and take it to have it notarized, a bonus if it is *free*. I will be out of town for a few days and my husband is away as well. Thanks in advance.

You can go on line and print off a form. here are a few I found..

If those dont work just do a google search for online Authorization to Consent to Medical Care forms. And you don't have to have them notarized. Just a signature from you works fine.

Hi K., we got ours from our doctors office just ask them if your's doesn't have one you can write it up your self we have done that before.just put your childs full name b-day allergies, medications they are on. give insurance imformation contact numbers, any history of surgerys and dates. giving permission to who ever you are having take care of your children full permission to act in your place until you can arrive. hope that helps.~L.~

It can be as easy as typing it on your computer if you'd like and take it to the local UPS store and have it notarized. Pretty simple don't cha think! I did this and it was ok!
