The couple's registry items are terrible. I know she doesn't cook, and he didn't go when she registered. They are late 20's, both work. He's outdoorsy, she's more athletic. Don't want to get a "run of the mill" gift. Gimme all of your ideas, please?
Well, I ended up buying a firepit on clearance. It was a very nice one, and she was thrilled. I had no idea she was receiving a patio set, so the firepit was a HUGE hit.

How about a picnic basket full of crumpled up dollar bills.
I hate to burst your bubble but when people register for gifts it means that is what they want! It drove me crazy when I got married and people tried to think of some unique gift that would stand out from the crowd. The gifts I remember and appreciated most were the gifts that were on our list (I am the opposite of the poster who cherished the engraved gifts because that is not my kind of thing so make sure you know them well enough to wander off the list of what they actually want). The rest ended up in boxes unused until I finally got up the guts to get rid of them. My vote is stick to the list or give them money/gift certificate to the place they are registered so they can fill in the things that don't get purchased by guests. Don't judge someone's registry choices, that is just not very polite. And if hubby to be is complaining that he didn't get any input to the list, he should be able to go and add on to the list.

or... quite honestly I don't think she'd register for something if she didn't want it. And if he really cared what she registered for, he'd have gone with her. Perhaps he looked over her choices online and you don't know it.
Still don't want to give a regular gift? How about something unique like "flowers for a year" a new bouquet each month? Oh wait you said $50. How much for those 'beers of the month' club things?
If she doesn't cook, how about an easy cookbook? I wouldn't be surprised if they had a 'Cookbook for the new Couple" book. Or perhaps get a casserole dish off her registry and then give her your family's favorite recipes in a cute recipe book.
My favorite thing to give is a set of t-shirts that say "I love my husband" and "I love my wife". What a wonderful thing for them to wear on their honeymoon! It's unique and they will be able to celebrate their new marriage and share their enthusiasm with others who've maybe lost the "spark" after being married for a while. You can find them on the web by doing a google search. And to be completely honest, I sell them on my website too! :-)
I hesitate to suggest this because it is my business, but I make establishment signs for weddings. What about a backyard volleyball net? Or, if they're athletic maybe you can get them a 2 pack of Power Bands ~ they are these rubber & magnet bracelets that are all the rage right now in the sports world.

Battery operated fly swatter
George Forman indoor grill with removable plates (easy to use, very healthy, and cooks food quickly)
Flannel bed sheets for cold months
A new tent, sleeping bags or air mattress is good for newlyweds - air matress is great if they are just starting out and may not have a "guest bed."
Anything engraved with a name or special customized saying is always unique. Get one of their favorite outdoor or athletic gadget and personalize it.
Do you have a place in your area that prepares dinners for people (not a restaurant but like a personal chef type place?) Or maybe hire a personal chef for them for one night as a gift certificate.
i cherish the items that are engraved the most, a picture frame with our last name & year on it, or monogrammed towels etc
If they are outdoorsy maybe you could get some sort of camping gear. Maybe a propane grill that can be used both camping and tailgating.
