Does she drink wine at all? This is a wonderful gift...absolutely delicious even tho it sounds gross. Maybe add a small box of truffles. You can't go wrong with wine & chocolate!

I would give her a special day out with you. If you have a favorite place you like to go, take her out for a coffee or lunch and a movie together. Let her know how special and important she is as a friend and how much you value your friendship with her. I know that as I am getting older, I value my girlfriends more and more and love to spend time with them - just being me, lol and not MOM. ;)
I just celebrated few of my few friends who turned 40 years old within one year!!
Few of my girlfriends and I surpriised a birthday gal with mini book telling a story that we will take her out all day next day. We took a birthday gal out for day spa or manicure/pedicure and lunch together!! We chipped her day expenses along with our own expenses.
We surprised one of my birthday girl one new makeover room at her house.
Few of my girlfriends and I made birthday gal favoite theme- a Zebra with pink gift basket with Zebra Bath wrap, Zebra soap, Zebra sleeping mask, Zebra manicure and pedicure zipper set, Zebra wallet and purses with her favorite treat/wine.

Okay - is there something she wish she had done by the age of forty? Could you give her a book about it?
Can you find a picture of her as a baby (family?) and have it blown up & framed? Maybe make a CD of 80s music she liked or something else nostalgic. Good luck!
If you have some fun photos of things you've done together you could have a calendar made for her and use cute captions. Example: if she/you all are drinking wine in a picture you could say, "Like a fine wine, you're not getting older--you are becoming more delicious!" Or similar.
This idea came from my brother, who made a calendar for me for my 50th (time flies, ouch) using photos of me, my siblings and parents from when we were very young. I still treasure it!
When one of my friends turned fifty, five of us thought of 10 things each about the birthday girl and we typed them up on strips of paper and put them in a pretty wooden box (I think it had a place for a picture on the top). She had to pull out the strips of paper, read it, and then tell us who typed it. It was things like..."I've seen more movies with you than with my husband!" or "You always listen to what I say. But more importantly, you hear what I don't say." It was so much fun for her to guess who said what, and for her to hear how much she is loved.
